1. Your valid passport ( Su pasaporte;lido ).You must have some form of photo ID. (Usted debe tener cierta forma de identificacin de la foto.)

2. Your vaccinations ( childhood and adult immunizations).

Written proof is required. New requirements are effective December 14, 2009. Bring a copy of your immunizations, receive immunizations at our office or have additional blood drawn for laboratory testing of immunity

3. If you are pregnant, we will need a letter from your OB physician stating your due date.(Si usted es embarazado, necesitaremos una letra de su medico de OB que indica su fecha debida.)

4. If you are breastfeeding, the CDC states that it is safe for you to receive immunizations. You may prefer to discuss this with your own physician prior to coming. 5. If you have had a previously positive tuberculosis skin test or have been treated for tuberculosis, please inform our staff and bring that documentation with you. 1. What is included in the cost of the examination ?
The price of examination includes the consultation about required vaccinations, tuberculosis skin test, syphilis blood test,physical exam by the physician and completion of form I-693. It does not include vaccinations as there are different vaccination requirements for each age group.The price is slightly higher if you choose the tuberculosis blood test in place of the tuberculosis skin test. The price for children, under 15, is lower because no syphilis blood testing is required.
2. How long does the examination take? 
This is a TWO-PART process. The initial visit is approximately 45-60 minutes per person to allow time for paperwork, speaking with health professionals and laboratory testing. For the return visit, please allow time for the examination, review of labs, questions and answers as well as sealing of forms in the required envelope for INS.
3. Should I bring my vaccinations with me to the office?
Yes. You may have had some or all of the required vaccinations. They will be reviewed.
4. What happens if I need vaccinations?
Many of the vaccinations are offered in our office. You will have the choice to get them in the office, at your physician's office or the health department. You will be given a list of the required ones. They can also be found above in the CDC link.
5. What other alternative is there to getting the vaccinations?
We can do IGG testing which is testing your blood to see if you are immune to a particular disease. If you have been previously vaccinated or have had the disease, your blood should show antibodies. That written lab test is adequate proof for INS and will be attached to your I-693 form.
6. What tuberculosis testing is required?
INS requires EITHER the tuberculosis skin test OR blood assay test. Please allow 2-3 business days for both results.
7. Which tuberculosis test should I choose?
If you have been previously vaccinated with the BCG vaccine, you may prefer to have the blood assay test. Many BCG vaccinated clients have a positive skin test reaction which requires a follow-up chest x-ray. The category of "class B latent "; may then be marked on the I-693 form. A recent addition to the I-693 form is the blood assay test, which is very specific. This is a test for the tuberculosis infection in the blood. The skin test may possibly be positive due to an allergic reaction on the skin. You MUST return in 2 - 3 days to have the skin test read. You DO NOT have to return in 2-3 days for the blood test. The CDC encourages all positive skin tests, despite negative chest x-ray results, to be referred to the health department.
8. Can I use prior syphilis results and chest x-ray results?
No. INS requires current testing with the civil surgeon.
9. If I am pregnant, how is my exam different?
You are still required to have the same testing. Some vaccinations are contraindicated - see above link. If your tuberculosis skin test is positive, INS still requires a chest x-ray.
10. If I have had the exam done before, is it still good and can I use it?
The exam expires after one year. The vaccination information can be used. The blood tests for syphilis and tuberculosis as well as the skin test are also expired and need to be repeated. If your skin test was positive, you may choose to have the blood assay or chest x-ray as stated above.
11. How will I receive my completed I-693?
You will receive a sealed envelope that you submit to INS or your attorney.
12. When will I receive the sealed envelope?
Once you have had your exam, blood tests and vaccination requirements met, you will get the envelope. The process usually takes 3-5 business days. Please make sure to complete this process within 30 days and submit your envelope to INS before it expires. You will receive a copy for your records.
